Tax Exempt Rules and Regulations Agreement

1. ) The taxable items purchased under this tax exempt account, or on any order or invoice related to this account, will be resold, rented, or leased by me within the geographical limits of the United States of America, its territories and possessions, or within the geographical limits of the United Mexican States, in their present form or attached to other taxable items to be sold.

2.) I understand that if I make any use of the items other than retention, demonstration or display while holding them for sale, lease or rental, I must pay sales tax on the items at the time of use based upon either the purchase price or the fair market rental value for the period of time used.

3.) I understand that it is a criminal offense to give a resale certificate to the seller for taxable items that I know, at the time of purchase, are purchased for use rather than for the purpose of resale, lease, or rental and, depending on the amount of tax evaded, the offense may range from a Class C misdemeanor to a felony of the second degree.

4.) I have verified with my local, state, or federal tax authority that I have an active and current account.

5.) I understand all federal, state, and local tax laws regarding the purchasing of the merchandise under this tax exempt account. I will be held liable and pay any taxes due, if any, (local, state or federal) for all purchases made with a tax exempt account and hold Soul Heart Hands, LLc. and all its subsidiaries harmless from any representations I make in this electronic form and paper tax exempt certificate. All taxes due will be payable to Fiesta At North Star® or Soul Heart Hands, LLC. or your local tax authority if applicable if any taxes are due from any state or country.

6.) This account is not transferrable and may not be used by anyone outside of the company or institution registered. All changes of tax status with the government, change of address, or name will make the use of this tax exempt account status illegal. Please notify us as soon as possible if any changes occur to update the information or cancel tax exempt status to this account.

7.) I further understand that I will be liable and pay a$200 per transaction penalty (payable to Soul Heart Hands, LLC.) for any violation or misuse of a tax exempt status account. If the stated tax exempt status information is found false or untrue criminal charges will be processed and a $500 penalty will be charged to my account to collect any taxes due.

8.) If items are shipped within Texas or the transaction is made in Texas, I will provide fully filled and signed copy of form 01-339 "Texas Sales and Use Tax Resale Certificate."

9.) I understand that this "Tax Exempt" status may be denied at Fiesta At North Star®'s discretion before items are shipped if the information provided is insuffient or not verifiable against government records. Taxes may be applied and charged by Fiesta At North Star® after online transaction is generated or items are shipped out. There are no time limits

Under penalties of perjury, I swear or affirm that the information provided on this form is true and correct as to every material matter. I also affirm to abide by the rules and regulations provided.

By submitting this registration and marking the checkbox on the form I am agreeing to all of the above terms and state that I have read them and understood them completely.
